At SMI, we accept most PPO insurance plans for Physical Therapy services. However, if you currently have an HMO, we offer cash options in order to meet your Physical Therapy needs. Below is a list of our cash prices. Call us for more information!

Physical Therapy Services:

Physical Therapy Evaluation and Treatment: $150 initial session only

Includes: An in-depth injury evaluation, including evaluation of contributing body segments and movement evaluation, manual therapy to area of injury and contributing body segments, therapeutic exercises and Plan of Care

Follow-Up PT 4 Session Package: $360 ($90 per session)

Follow-Up PT 8 Session Package: $680 ($85 per session)

Injury Screen

One Body Part Screen: $25

Includes 1 body part injury assessment with minimal manual therapy & advice on next step

Other Services:

Cupping and/or Scrapping: $20 per 15 min

Taping: $5-10 Depending on type of tape used & body part

Sports Massage: $65 per 30 min
